DUE TO LIMITED SPACE, ASA REQUIRES THAT ALL FORMS RELEVANT TO YOUR PROGRAM BE RECEIVED WITHIN ONE MONTH AFTER THE APPLICATION IS RECEIVED. FAILURE TO SUBMIT THESE FORMS MAY JEOPORDIZE YOUR PLACE AND DEPOSIT.
STEP 1 — APPLICATION
Complete the online application or downloaded application form. If applying online, you must download and print the Parent/Student Agreement, sign it, and return it to ASA with your application.

STEP 2 — PROGRAM DEPOSIT
You must submit a $1,000 program deposit with your application.
Program deposits can be paid by check, by Visa or Mastercard (Visa only for non-US students), or by wire transfer (telex). Please mail checks to ASA Admissions, 375 W Broadway Rm 200, New York, NY 10012-4324. Credit cards may be used in the online application, or faxed or phoned in to the ASA office. Applications cannot be processed without the program deposit.
Applications submitted after April 1 must include full payment, refundable only if you are not accepted to the program.
STEP 3 — SUPPLEMENTAL FORMS
All supplemental forms are available for download as well as in print in each brochure. Completed supplemental forms and all other supporting materials should be mailed or faxed to:

Teacher/Counselor Recommendation
All applicants to an ASA program must submit a completed teacher or counselor recommendation form.

School Transcript/Grade Report
All applicants to an ASA program must submit a copy of their school transcript, or, if currently in the 9th grade, most recent grade report. Students applying to College Admissions Prep programs must also request a SCHOOL PROFILE to accompany the transcript.
There is no form for a school transcript, grade report or profile. These items should be requested from the school and sent directly to ASA.
Homestay Questionnaire/Phone Interview — Spanish in España applicants
All applicants to an ASA homestay program must complete and submit a homestay questionnaire as part of the admissions process. After we have received your application, parent/student agreement, teacher recommendation, transcript, and homestay questionnaire, please contact the ASA office to schedule an interview.

CAP Parent Survey
All students applying to the ASA College Admissions Preparation (CAP) programs will need to have a parent or guardian complete and submit the CAP Parent Survey. Forms must be submitted to ASA to hold a place on the program of choice.
Completed applications will be reviewed on a rolling basis. There is no final date for application. Availability will depend upon residential space and enrollment in particular courses.

A program deposit must be submitted with your application. The full program deposit is credited to the total tuition fee. An additional $1000 is due upon acceptance to secure your place in the program and in the courses of your choice. The balance of the fee is due by April 1. Applications submitted after April 1 must be accompanied by payment of the full tuition fee, refundable only if you are not accepted to the program.
Facility/Damage Deposit
All students will be charged for a $300 security deposit when invoiced for full program costs. This security deposit is distinct from the above program deposit. Security deposits are intended to cover lost keys and room damages. Security deposits, minus charges, will be returned after completion of the program and when ASA has received a full accounting from the business offices of the universities, usually by the end of September.
Payment Methods:
Deposits for applications submitted on line can only be paid by credit card (Mastercard or Visa; Visa only for non-US cardholders). Subsequent payments may be made by credit card, check, or wire transfer. Detailed payment options will be provided with each program invoice.
By Credit Card (Visa and Mastercard only):
- Visa and Mastercard credit cards may be submitted through the online application, over the phone, or by fax.
- Only Visa is accepted for non-US students.
- A service fee of $25 will be charged for any cards rejected for payment.
By Check:
- Checks must be drawn on a US bank and must be in US dollars.
- Payable to Academic Study Associates.

- Note student's name on all checks.
- A service fee of $25 will be charged for any returned checks
By Wire Transfer (Telex):
- Contact ASA for bank details.
- Reference: Name of student.
- Please fax a copy of the transaction to: Academic Study Associates at (+1) 212-334-4934 .
- Bank charges incurred are the responsibility of the parent.
- ASA will credit the student's account with the actual amount in US dollars that has been credited to its account.
Program Fee Includes
Tuition; room; three meals per day (except as below); books, except college credit courses; some art and photography supplies; guest lectures; class related field trips; use of select UMass Amherst, UC Berkeley, and Oxford campus facilities; the complete academic and excursion program as described in the brochure, except those marked as optional; all afternoon and evening activities, except those marked as extra, or held off campus and not organized by ASA; ASA lanyard and yearbook; linen, bedding, towels; transfer to and from Bradley Airport, Oakland Airport or San Francisco Airport; transportation between airport and program for students traveling on European ASA escorted flights.
Program Fee Does Not Include
Transportation to/from your home city; lunches at Oxford, Barcelona and Florence; courses and activities shown to have an extra fee; books for college credit courses; stationery; laundry; afternoon and evening activities held off the host campus and not directly run by ASA; snacks, souvenirs, pocket money or meals eaten away from the program; trip cancellation insurance (Travel Insurance and Assistance Plan); medical insurance, medical and dental fees, and prescriptions.
If a student reserves a place on an ASA program and subsequently withdraws, the respective refund policies will apply.

Notification of withdrawal must be made in writing to ASA Admissions, 375 W Broadway Rm 200, New York, NY 10012. Withdrawals are effective on the date that ASA receives written notification. Students who have reserved a place on an ASA program before February 1 (Language Immersion and UK Pre-College) or March 15 (US Pre-College, CAP) but have not received a final acceptance by that date because of missing documents are still subject to the above-stated refund policy. Students who have enrolled in an optional excursion may withdraw without penalty up until June 1. After that date, students will be held responsible for the full payment of the excursion.
There is no refund of the Travel Insurance and Assistance Plan fee. There will be no refunds for students who voluntarily withdraw for any reason from an ASA program, nor will there be any refund for a student who is asked to leave a program for the use of drugs or alcohol, non-notification of a serious medical condition, or gross disobedience of the rules of the program (decisions which are at the sole discretion of ASA). Dates, fees, itineraries and courses may be changed without notice should circumstances require it. Academic Study Associates reserves the right to cancel any program and to decline to accept any person as a member of one of its programs. If ASA cancels a program, the full program fee will be refunded.

The following materials are needed to support each ASA application. Please download all the forms relevant to your program, complete them and return them to the ASA office. All supplemental materials must be received within one month after initial application.

Travel Insurance
ASA offers an optional Travel Insurance and Assistance Plan for your protection should you have to cancel or interrupt your participation in a program. Information on this plan will be sent with both the acknowledgment and acceptance letters. You can also view complete policy details at: http://www.tripmate.com/wp433A/.
In order to gain the full benefits from this Plan, you must indicate your acceptance of the fee within two weeks of receiving your acceptance letter from ASA. You must be a US/Canadian citizen or permanent resident to be eligible.
Medical Expenses/Medical Insurance
Parents are responsible for all costs of medical and dental care, tests, allergy shots and prescriptions. Parents must complete an ASA Medical Form, detailing complete health history of the student and giving their Health Insurance Carrier information. All bills from health facilities used by ASA will be sent directly to parents.
Travel Visas
All ASA students are responsible for obtaining all travel documents (including visas) necessary to attend an ASA program. Non-US passport holders who require a visa for travel to an ASA program should, due to the nature of each program and its duration, obtain the equivalent of a tourist visa, not a student visa, from the appropriate embassy or consulate.
Payment
Non-US residents may pay for ASA programs by credit card (Visa only), wire transfer, or check (in US dollars drawn from a US bank). Details related to forms of payment will be included with program invoices.
Medical Insurance
All ASA students are responsible for making sure that they are properly insured for health care during their participation in an ASA program. Please contact the ASA office for more information.
Transportation To and From ASA Programs in the US
ASA staff will meet students at the following airports within the specified arrival day / departure day time windows at no additional cost. Students will be met at the baggage claim arrivals area and ASA will transport students between the airport and the program campus.

ASA cannot guarantee transfers to and from program airports if your flight plans differ from the arrival/departure day time frames listed above. Students arriving at or departing from airports other than those cited above are responsible for their own transportation costs.
Group Flights for European Programs
ASA offers escorted transatlantic flights on regularly scheduled airlines. All students traveling on ASA escorted flights will be escorted on their international flight and transferred to and from the airport on their arrival and departure. Students attending the Oxford experience program not traveling on the escorted flight will pay a supplemental fee for transportation from the airport to the program. Upon return to the US, students are released from the program at the point of entry once they have cleared customs. Full information about ASA’s escorted flights will be sent to enrolled students when available. Please note that students are responsible for booking their own seats on ASA escorted flights.
